1095-B Tax Form

CHIP will mail 1095-B tax forms to all households whose children had CHIP insurance during the calendar year 2016. Forms must be mailed by March 31, 2017. The 1095-B tax form is being mailed because IRS regulations state that all health insurance administrators must send this form to all enrollees that had health insurance that meets the Minimum Essential Coverage requirements of the Affordable Care Act. 

If you have changed your physical address but have not reported the address change to your insurance contractor you will not receive your form due to an incorrect address.  Report your address change to your health insurance contractor and ask them to regenerate a new form.
