Applying

Applying for CHIP is easy and can be done in a variety of ways – online, phone or mail; whatever you feel most comfortable with. Before you start the application process, there are some important pieces of information you should have handy. Having this info readily available will make the process must faster. You should have:

  • Income amounts for your entire household before taxes - this includes income from employment and all other forms of income (for example - social security, pension, workers' compensation, unemployment, child support, etc)
  • Social Security Numbers and birthdates for all applicants
  • Day care expenses for your household (if any)
  • Work transportation expenses for your household (if any)
  • Private health insurance information (if you have or had private health insurance in the last six months)

Renewing

If you're already enrolled in CHIP, you'll need to renew every year to continue your child's coverage. Like the initial application process, some information is needed in order to complete the renewal process. Have ready:

  • Renewal Notice from your CHIP insurance company
  • Income amounts for your entire household before taxes - this includes income from employment and all other forms of income (for example - social security, pension, workers' compensation, unemployment, child support, etc)
  • Social Security Numbers and birthdates for all applicants
  • Day care expenses for your household (if any)
  • Work transportation expenses for your household (if any)
  • Private health insurance information (if you have or had private health insurance in the last six months)

My child has CHIP health coverage. Will anything be changing with the new federal health care reform law?
Current CHIP enrollees will continue to receive coverage through CHIP. However, after 2014, there will be some changes in income eligibility for CHIP and for Medical Assistance for children 6 through 18 years of age. Some children who previously qualified for FREE CHIP will then qualify for Medical Assistance.
